As reações adversas podem ser semelhantes aos sintomas de depressão. Portanto, às vezes é difícil diferenciar quais são os sintomas resultantes da própria doença daqueles devidos ao tratamento com Remeron. As reações adversas mais frequentes durante o tratamento com Remeron são: aumento de apetite e de peso; sonolência/sedação, geralmente durante as primeiras semanas de tratamento (Nota: Em geral, a diminuição da dose não reduz a sedação e pode comprometer a eficácia antidepressiva). Raramente, podem ocorrer as seguintes reações adversas: hipotensão ortostática; mania; convulsões; tremores; edema e aumento de peso; depressão aguda da medula óssea (eosinofilia, granulocitopenia, agranulocitose, anemia aplástica e trombocitopenia);
